Mental health and housing partnerships
From initial exploratory conversations and engaging relevant stakeholders to facilitating roundtables and developing strategies, we have supported NHS Trusts to integrate mental health services with local housing providers.
Clients include Dorset HealthCare University Foundation Trust, SW London St Georges, Greater Manchester Mental Health Trust.
Complex care pathways (including integrated step-down complex care pathway)
We work with NHS Trust leaders to review all aspects of adult mental health services, creating a pathway that identifies where greater integration with housing for people with complex care will produce best results.
Clients include: Southwark/SLAM/South London Partnership, Sussex Partnership.
Hospital discharge protocol
We work with NHS Trusts to help them create and test hospital discharge protocols for addults with learning disabilities and autism, so they can be supported to move into community-based supported housing.
Clients include: NHS Midlands – Birmingham and Solihull ICS, Bedfordshire, Luton and Milton Keynes ICS.
Care leavers protocol, pathway and housing
We work with NHS Trusts to explore how to integrate housing into the 16+ care leavers pathway.
Clients include: West Sussex County Council.
We develop collaborations between NHS partners and local housing associations so the latter can support their tenants and local people into local health jobs.
Clients include: Cheshire and Merseyside Health and Care Partnership
We support mental health NHS Trusts to engage with local housing providers around the development of new services in an existing NHS Trust property.
